Spectinomycine binds to 30S subunit of the bacterial ribosome, thus inhibiting protein synthesis. It has modest activity against a wide range of gm+ve and gm-ve organisms, though anaerobic organisms are mostly resistant.

Spectinomycine is an aminocyclitol antibiotic produced by a species of soil microorganism designated as Streptomyces spectabilis. In vitro studies have shown spectinomycin to be active against most strains of Neisseria gonorrhoeae (minimum inhibitory concentration <7.5 to 20 mcg/mL). Footprint studies indicate that spectinomycin exerts regional effects on ribosomal structure.

Uses

Spectinomycine Sterile Powder is used for the treatment of:

  • Acute gonorrheal urethritis and proctitis in the male
  • Acute gonorrheal cervicitis and proctitis in the female
  • Men and women with known recent exposure to gonorrhea should be treated as those known to have gonorrhea.

How Spectinomycine works

Spectinomycine is an inhibitor of protein synthesis in the bacterial cell; the site of action is the 30S ribosomal subunit. It is bactericidal in its action.

Dosage

Adults (Men and Women): Inject 5 ml intramuscularly for a 2 gm dose.This is also the recommended dose for patients being treated after failure of previous antibiotic therapy

Intramuscular injections should be made deep into the upper outer quadrant of the gluteal muscle.

Spectinomycine Sterile 2 grams Powder: Reconstitute with 3.2 ml of the accompanying water for injection with 0.9% Benzyl alcohol. Shake vials vigorously immediately after adding diluent and before withdrawing dose.

Side Effects

The following reactions were observed during the single dose clinical trials: soreness at the injection site, urticaria, dizziness, nausea, chills, fever and insomnia

Toxicity

Acute oral toxicity (LD50): >5000 mg/kg [Rat]. Information on overdosage in humans is not available.

Precaution

The usual precautions should be observed with atopic individuals. Prescribing Spectinomycine Sterile Powder in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to provide benefit to the patient and increases the risk of the development of drug-resistant bacteria

Interaction

May result in lithium toxicity when used concurrently. May increase or prolong the neuromuscular blocking effect of atracurium.

Half Life

1 to 3 hours in patients with normal renal function and 10 to 30 hours in patients with impaired renal function with a creatinine clearance < 20 mL per minute.

Pregnancy & Breastfeeding use

Pregnancy Category B. Since there are no controlled studies of Spectinomycine in pregnant women, and because animal reproduction studies are not always predictive of human responses, Spectinomycine should be used during pregnancy only if clearly needed.

Nursing Mothers It is not known whether this drug is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when Spectinomycine is administered to a nursing woman.

Contraindication

The use of Spectinomycine Sterile Powder is contraindicated in patients previously found hypersensitive to it.

Acute Overdose

Information on overdosage in humans is not available. Hemodialysis has been reported to aid in the removal of intravenously administered Spectinomycine from the body.

Storage Condition

Store between 20-25°C. Reconstituted solution should be used within 24 hr after preparation.
