Phenylephrine is a sympathomimetic with mainly direct effects on alpha-adrenergic receptors and weak beta-adrenergic activity. It causes vasoconstriction of the arterioles of the nasal mucosa and conjunctiva; activates the dilator muscle of the pupil to cause contraction; produces vasoconstriction of arterioles in the body and produces systemic arterial vasoconstriction.

Tetryzoline is a sympathomimetic with alpha-adrenergic activity with little or no effect on beta-adrenergic receptors.

Zinc Sulphate: Cofactor in over 100 enzymes; plays a role in DNA synthesis; supports a healthy immune system; helps maintain a sense of smell and taste; may assist in proper function of insulin.

How Oculoforte works

Phenylephrine is an alpha-1 adrenergic agonist that mediates vasoconstriction and mydriasis depending on the route and location of administration. Systemic exposure to phenylephrine also leads to agonism of alpha-1 adrenergic receptors, raising systolic and diastolic pressure as well as peripheral vascular resistance. Increased blood pressure stimulates the vagus nerve, causing reflex bradycardia.

Dosage

Oculoforte dosage

Adult: Instill 1 to 2 drops in the affected eye(s) up to 4 times daily.

Child >6 years: Instill 1 to 2 drops in the affected eye(s) up to 4 times daily

Side Effects

Blurred vision; minor stinging or tingling when the medicine is dropped into the eye. Severe allergic reactions (rash; hives; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); changes in vision; eye pain; worsening or persistent eye irritation or redness.

Toxicity

Patients experiencing and overdose may present with headache, hypertension, reflex bradycardia, tingling limbs, cardiac arrhythmias, and a feeling of fullness in the head. Overdose may be treated by supportive care and discontinuing phenylephrine, chronotropic medications, and vasodilators. Subcutaneous phentolamine may be used to treat tissue extravasation.

Precaution

Sympathomimetics are generally contraindicated in patients with narrow-angle glaucoma. Contact lenses should be removed before use. Patients should be instructed to avoid allowing the tip of the container to contact the eye, eyelid, fingers or any other surface. Overuse may result in rebound congestion. Patients should be advised to discontinue use and notify their physician if their condition worsens or lasts for more than 72 hours.

The volume of distribution of phenylephrine is 340L.

Elimination Route

Phenylephrine is 38% orally bioavailable. Clinically significant systemic absorption of ophthalmic formulations is possible, especially at higher strengths and when the cornea is damaged.

Half Life

Intravenous phenylephrine has an effective half life of 5 minutes and an elimination half life of 2.5 hours.

Clearance

Phenylephrine has an average clearance of 2100mL/min.

Elimination Route

86% of a dose of phenylephrine is recovered in the urine with 16% as the unmetabolized drug, 57% as the inactive meta-hydroxymendelic acid, and 8% as inactive sulfate conjugates.
