সেকোক্লাভ

Cefuroxime is one of the bactericidal second generation cephalosporin antibiotics, which is active against a wide range of Gram-positive and Gram-negative susceptible organisms including many beta-lactamase producing strains. It is indicated for the treatment of infections caused by sensitive bacteria.

Clavulanic acid has a similar structure to the beta-lactam antibiotics but binds irreversibly to the beta-lactamase enzymes.

The presence of clavulanic acid in Cefaclav formulations protects Cefuroxime from degradation by beta-lactamase enzymes and effectively extends the antibacterial spectrum of Cefuroxime to include many bacteria normally resistant to Cefuroxime and other cephalosporins.

• নাক, কান, গলার বিভিন্ন সংক্রমণ যেমন - মধ্যকর্ণের প্রদাহ, সাইনাস সমূহের প্রদাহ, টনসিলের প্রদাহ এবং ফ্যারিংস এর প্রদাহ।
• তীব্র এবং দীর্ঘস্থায়ী ব্রংকাই এর প্রদাহ এবং নিউমােনিয়া।
• ফারানকুলােসিস, পায়ােডার্মা এবং ইমপেটিগাে।
• পায়েলােনেফ্রাইটিস, মূত্রনালী এবং মূত্রথলীর সংক্রমণ।
• তীব্র কিন্তু জটিল নয় এমন গণােকক্কাল ইউরেথ্রাইটিস ।
• প্রাথমিক লাইম ডিজিজ এবং পর্যায় ক্রমিক লাইম ডিজিজে প্রতিরােধক হিসেবে।

সেকোক্লাভ খাওয়ার নিয়ম / ব্যবহারের নিয়ম

• কিশাের কিশােরী এবং প্রাপ্ত বয়স্কদের জন্য (১৩ বৎসর ও তার অধিক) : ২৫০ অথবা ৫০০ মি.গ্রা., প্রতিদিন দুইবার করে ১০ দিন।
• শিশুদের ক্ষেত্রে (যারা পূর্ণ ট্যাবলেট গ্রহণ করতে পারে) : ২৫০ মি.গ্রা. প্রতিদিন দুইবার করে ১০ দিন।
• শিশুদের ক্ষেত্রে (৩ মাস থেকে ১২ বৎসর) : ২০-৩০ মি.গ্রা./কেজি/দিন দিনে ২ বার অথবা চিকিৎসকের পরামর্শ অনুযায়ী ব্যবহার্য।

Cefuroxime & Clavulanic Acid may be administered without regard to meals.

পার্শ্বপ্রতিক্রিয়া

সাধারণত সেফুরক্সিম ও ক্ল্যাভুলেনিক এসিড সুসহনীয়। তদুপরি কিছু পার্শ্বপ্রতিক্রিয়া যেমন - বমি বমি ভাব, বমি, ডায়রিয়া, পেট ব্যথা বা অস্বস্তি হতে পারে।

সতর্কতা

পােটেন্ট ডাইইউরেটিক সেবন করছেন এবং সিউডােমেমব্রেনাস কোলাইটিসের ইতিহাস আছে এমন রােগীদের ক্ষেত্রে সেফুরক্সিম সতর্কতার সাথে ব্যবহার করতে হবে।

মিথস্ক্রিয়া

Probenecid: Concomitant administration of probenecid with Cefuroxime increases the area under the serum concentration versus time curve by 50%. The peak serum Cefuroxime concentration after a 1.5 gm single dose is greater when taken with 1 gm of probenecid than without probenecid.

Antacids: Drugs that reduce gastric acidity may result in a lower bioavailability of Cefuroxime and Clavulanic acids compared with that of fasting state and tend to cancel the effect of postprandial absorption.

Oral contraceptives: In common with other antibiotics, Cefuroxime may affect the gut flora, leading to lower estrogen re absorption and reduced efficacy of combined oral estrogen/ progesterone.

গর্ভাবস্থাকালীন ব্যবহার

• গর্ভাবস্থায় প্রথম তিনমাস সাধারণত সকল এন্টিবায়ােটিক পরিহার করা উচিত। তদুপরি মূত্রতন্ত্র ও অন্যান্য সংক্রমণের ক্ষেত্রে গর্ভাবস্থার শেষ দিকে নিরাপদভাবে ফ্যামিক্ল্যাভ ব্যবহার করা যেতে পারে।
• স্তন্যদানকালে ও ফ্যামিক্ল্যাভ মাতৃদুগ্ধের সাথে স্বল্পপরিমানে নিঃসৃত হয়। তদুপরি শিশুর দেহে সংবেদনশীলতার সম্ভাবতার কথা মনে রাখা উচিত।

বৈপরীত্য

সেফালোসপােরিন এর প্রতি সংবেদনশীল রােগী ও সিউডােমেমব্রেনাস কোলাইটিসে আক্রান্ত রােগীদের জন্য প্রতিনির্দেশিত।

অতিরিক্ত সতর্কতা

তীব্র ওভারডোজ

Signs and symptoms: Overdosage of Cefaclav can cause cerebral irritation leading to convulsions.

Management: Serum levels of Cefaclav can be reduced by haemodialysis and peritoneal dialysis.

অন্যান্য ওষুধের সাথে প্রতিক্রিয়া

প্রবেনেসিডের সাথে একত্রে ব্যবহার সময়ের প্রেক্ষিতে ফ্যামিক্ল্যাভের সিরাম ঘনত্ব ৫০% বৃদ্ধি করে। যে সমস্ত ঔষধ গ্যাস্ট্রিক এসিডিটি হ্রাস করে সেগুলাে ফ্যামিক্ল্যাভের বায়ােএ্যাভেইল্যাবিলিটি হ্রাস করে এবং খাবারের পর শােষণের প্রভাব নির্মূল করতে পারে।
